Complete your practice details on your phone

The practice form uses one column on mobile. Work down the page before continuing.

  1. Enter your practice and contact details

    Check the practice name and speciality, add your optional logo and complete the contact fields.

  2. Enter the address, then your search radius

    The address fields and radius are stacked. The postcode should match the address you enter.

  3. Continue after completing the form

    Scroll to the bottom and choose Continue to people. If validation highlights a field, correct it before continuing.
